The Lasting Power of Attorney (LPA) is a legal document that allows a person (donor) aged 21 years and above to appoint one or more person (donee(s)) to make decision on his behalf should he lose mental capacity to do so.

What are the criteria to making an LPA?

You need to be at least 21 years old, and have mental capacity. You will need to register the completed LPA Form 1 and other relevant documents with the Office of the Public Guardian (OPG).

What are the documents I need to bring along?

Please bring along your NRIC / Passport, and fill out the LPA Form 1.
